How to request

Okay all you have to do is put up a name which says your forum's name
For instance suppose i'm about to request affiliation what i'll do is
Put forum's name "Frinds club" cause it's the site which i want to affiliate
I hope i was clear?
& posts saying "Please affiliate my site" or anything like that will be IGNORED

Forum's Name : Your forum's/site's name
URL : Address of your forum/site
Affiliate image : The image i should add for your site/forum(Maximum Image Size limit is 120*63)
